Despite the considerable improvement in global health, millions of people still lack access to quality health services, including access to effective antimicrobial medicines, or are impoverished as a result of health spending. At the same time, antimicrobial resistance – a consequence of overuse a...nd misuse of antimicrobials – is increasingly a barrier to accessing effective care. The declining effectiveness of antibiotics is driven by multiple factors, many of which can be addressed through well functioning primary health care. However, primary health care has not always had much attention in national health sector responses to
antimicrobial resistance, which often focus on tertiary care, laboratory detection and surveillance. The three pillars of primary health care (community engagement, front-line health services including primary care and essential public health, and multisectoral action on wider health determinants) are central not just to Universal Health Coverage and the Sustainable Development Goals, but also to an effective response to antimicrobial resistance.

The evidence produced in mathematical models plays a key role in shaping policy decisions in pandemics. A key question is therefore how well pandemic models relate to their implementation contexts. Drawing on the cases of Ebola and in...fluenza, we map how sociological and anthropological research contributes in the modelling of pandemics to consider lessons for COVID-19. We show how models detach from their implementation contexts through their connections with global narratives of pandemic response, and how sociological and anthropological research can help to locate models differently. This potentiates multiple models of pandemic response attuned to their emerging situations in an iterative and adaptive science. We propose a more open approach to the modelling of pandemics which envisages the model as an intervention of deliberation in situations of evolving uncertainty. This challenges the ‘business-as-usual’ of evidence-based approaches in global health by accentuating all science, within and beyond pandemics, as ‘emergent’ and ‘adaptive’.

Ces lignes directrices provisoires offrent des recommandations sur les utilisations prioritaires des tests de diagnostic rapide à détection d'antigène (TDR-Ag) dans des populations et des contextes spécifiques, notamment (i) pour la détection primaire des cas chez les personnes symptomatiques suspectées d'être infectées et les personnes asymptomatiques à haut risque de COVID-19, (ii) pour la recherche des contacts, (iii) pendant les enquêtes sur les épidémies et (iv) pour surveiller les tendances de l'incidence de la maladie dans les communautés. Les TDR-AG répondant aux exigences minimales de performance peuvent être utilisés en dehors des milieux cliniques et des laboratoires, y compris dans les communautés, par des opérateurs formés conformément aux instructions. Segundo dados prelimi...nares de estudos observacionais, estima-se que aproximadamente 10% dos pacientes com quadros leves a moderados de COVID-19 apresentam sintomas prolongados, que duram 3 semanas ou mais [2]. Essa condição tem sido chamada de COVID “longa”, “pós-aguda” ou
“síndrome pós-COVID-19”. Entre os pacientes que necessitam internação, especialmente em UTI, estes sintomas residuais podem ser muito mais frequentes: mais que 80% dos pacientes reportaram ao menos 1 sintoma após 60 dias do início do quadro.

Key Findings: Exposure to vaccination information from faith leaders and health facilities was associated with increased likelihood of vaccination uptake. The significant association between exposure to a... greater number of immunization information sources and increased likelihood of vaccination uptake reinforces the need for multiple sources to provide consistent and accurate immunization information to facilitate positive vaccination behavior.
Key Implications: Social and behavior change communication interventions may optimize the promotion of immunization services through multiple information sources such as health facilities and community-based assets including faith leaders and lay community health workers. Religion and faith play an important role in how people understand health and make health decisions. In Sierra Leone and other similar settings, interventions to improve uptake of immunization services may be enhanced by proactively engaging faith leaders.
