The first chapter of the booklet begins with the latest estimates of the number of people who use drugs, the distribution of those users by type of drug...s, age and sex, and recent trends in the use of drugs. The chapter also reviews the impact of the coronavirus disease (COVID-19) pandemic on drug use patterns and service provision. Other issues examined in the chapter are the health consequences of drug use, including the number of people in treatment for drug use disorders and the extent of drug injecting and of HIV and hepatitis C among people who inject drugs. The chapter concludes with a review of the extent to which strategies, policies and interventions are in place to respond to the drug use problem.

An analysis of different eradication strategies is included, as well as of the role of women in the cocaine supply chain. The chapter also presents the latest estimates of and trends in cocaine use, including a brief introduction to the various cocaine consumer products. Continuing a worrying decade-long rising trend, the number of people forced to flee due to persecution, conflict, violence, human rights violations and events seriously disturbing public order climbed to 89.3 million by the end of 2021. This is more than double the 42.7 million people who remained f...orcibly displaced at the end of 2012 and represents a sharp 8 per cent increase of almost 7 million people in the span of just 12 months. As a result, above one per cent of the world’s population – or 1 in 88 people – were forcibly displaced at the end of 2021. This compares with 1 in 167 at the end of 2012. During 2021, some 1.7 million people crossed international borders seeking protection and 14.4 million new displacements within their countries were reported. This is a dramatic increase from the combined 11.2 million a year earlier.

During the first year of the Covid-19 pandemic, the world’s economy slowed. Yet, the global annual average particulate pollution (PM2.5) was largely unchanged from 2019 levels. At the same time, growing evidence shows air pollution—even when experienced at very low levels—hurts human health. T...his recently led the World Health Organization (WHO) to revise its guideline for what it considers a safe level of exposure of particulate pollution, bringing most of the world—97.3 percent of the global population—into the unsafe zone. The AQLI finds that particulate air pollution takes 2.2 years off global average life expectancy, or a combined 17 billion life-years, relative to a world that met the WHO guideline. This impact on life expectancy is comparable to that of smoking, more than three times that of alcohol use and unsafe water, six times that of HIV/AIDS, and 89 times that of conflict and terrorism.
