Addressing comorbidities and risk factors for TB is a crucial component of Pillar one of the End TB Strategy, which focuses on integrated patient-centred care and ..., including action on TB and comorbidities. The Framework for collaborative action on TB and comorbidities aims to support countries in the evidence-informed introduction and scale-up of holistic people-centred services for TB, comorbidities and health-related risk factors, with the goal of comprehensively addressing TB and other co-existing health conditions. It should be used in conjunction with relevant WHO guidelines. The Framework is intended for use by people working in ministries of health, other relevant line-ministries, policymakers, international technical and funding organizations, researchers, nongovernmental and civil society organizations, as well as primary care workers, specialist health practitioners, and community health workers who support the response to TB and comorbidities in both the public and private sectors. more
Objective: To identify gaps in national stroke guidelines that could be bridged to enhance the quality of stroke care services in low- and middle-income countries. Methods: We systematically searched medical databases ... websites of medical societies and contacted international organizations. Country-specific guidelines on care and control of stroke in any language published from 2010 to 2020 were eligible for inclusion. We reviewed each included guideline for coverage of four key components of stroke services (surveillance, prevention, acute care and rehabilitation). We also assessed compliance with the eight Institute of Medicine standards for clinical practice guidelines, the ease of implementation of guidelines and plans for dissemination to target audiences. Findings: We reviewed 108 eligible guidelines from 47 countries, including four low-income, 24 middle-income and 19 high-income countries. Globally, fewer of the guidelines covered primary stroke prevention compared with other components of care, with none recommending surveillance. Guidelines on stroke in low- and middle-income countries fell short of the required standards for guideline development; breadth of target audience; coverage of the four components of stroke services; and adaptation to socioeconomic context. Fewer low- and middle-income country guidelines demonstrated transparency than those from high-income countries. Less than a quarter of guidelines encompassed detailed implementation plans and socioeconomic considerations. Conclusion: Guidelines on stroke in low- and middle-income countries need to be developed in conjunction with a wider category of health-care providers and stakeholders, with a full spectrum of translatable, context-appropriate interventions. more

When antibiotics become ineffective, bacterial infections lead to increased morbidity, use of healthcare, mortality ... cost. Globally, estimates suggest that AMR leads to 700 000 deaths per annum. For the EU, the European Centre for Disease Prevention and Control (ECDC) has estimated that AMR currently causes 25 000 deaths annually and losses of at least EUR 1.5 billion per annum in extra healthcare costs and productivity. more

The primary audience for this guidance is persons working directly in vector-borne disease prevention and control, including programme managers, researchers ... field workers. A brief technical background is provided for the benefit of persons without expertise in vector-borne diseases; readers working in the field may wish to skip the background section and begin with the discussion of ethical issues and values in Chapter 3. The guidance cannot offer universally applicable answers to the complex ethical issues raised, nor can it provide a checklist of issues that are necessarily relevant in all situations. Rather, its goal is to help readers recognize aspects of their work that raise significant ethical challenges and to respond to these challenges in accordance with internationally accepted values and norms. more

Self-care interventions are among the most promising and exciting new approaches to improve health and well-being, both from a health systems perspective ... for people who use these interventions. The World Health Organization (WHO) uses the following working definition of self-care: Self-care is the ability of individuals, families and communities to promote health, prevent disease, maintain health, and cope with illness and disability with or without the support of a health worker. The scope of self-care as described in this definition includes health promotion; disease prevention and control; self-medication; providing care to dependent persons; seeking hospital/specialist/primary care if necessary; and rehabilitation, including palliative care. It includes a range of self-care modes and approaches. While this is a broad definition that includes many activities, it is important for health policy to recognize the importance of self-care, especially where it intersects with health systems and health professionals. more
