Scientific Brief 9 July 2020
Brochure on 'increase in the risk of violence against older people' and what can be done to adress it.
A recent study informed that ivermectin was successfully used in vitro for the treatment of SARS-CoV-2 in experimentally infected cells and two preprint publications on observational clinical studies reported the apparent utility of ivermectin to treat patients with COVID-19 needing mechanical venti...lation. However, neither of these studies were peer-reviewed nor formally published and one of them was later retracted.

Africa Centers for Disease Control and Prevention (Africa CDC) is aware of the media releases of the preliminary results of a large randomized clinical trial conducted in the United Kingdom, which included dexamethasone, a corticosteroid, as one of the drugs used for the treatment of COVID-19 patien...ts. The investigators reported that administration of oral or injectable dexamethasone resulted in about one-third reduction in mortality among COVID-19 patientsi that required mechanical ventilation and about one-fifth for patients requiring oxygen.

N95 or equivalent respirators are single-use personal protective equipment (PPE) designed for use by health workers that provide direct care to patients with diseases transmitted by aerosols or during aerosol generating procedures (AGP) for patients with acute respiratory disease, as is the case of ...COVID-19. • Given the current shortage of N95 and equivalent respirators, the World Health Organization (WHO) has suggested the possibility of their extended use by the same individual for up to 6 hours or the reprocessing of respirators when necessary.

In December 2019 a novel coronavirus (2019-nCoV) was identified as the causative agent of a severe acute respiratory illness among people exposed in a seafood market in Wuhan, China; • Human-to-human transmission has been documented, including in healthcare workers, and aerosol-generating procedur...es (AGP)† may play a role in the spread of the disease; • There are uncertainties in the natural history of the 2019-nCoV, including source(s), transmissibility mechanisms, viral shedding, and persistence of the virus in the environment and on fomites; • As of 6 February 2020, the following precautions are recommended for the care of patients with suspected or confirmed cases of 2019-nCoV: o For any suspected or confirmed cases of 2019-nCoV: standard + contact + droplet precautions o For any suspected or confirmed cases of 2019-nCoV and AGP: standard + contact + airborne precautions • The use of personal protective equipment (PPE) by healthcare workers requires an evaluation of the risk related to healthcare-related activities;

Most shelters in the Caribbean are community centers, schools, or churches that are limited in size. The novel coronavirus disease (COVID-19) distancing requirements subsequently reduced the number of persons a shelter can accommodate during the hurricane season. This document reinforces some measur...es to follow per international emergency shelter protocols factoring in conditions for spacing between beds/cots, recreation areas and ventilation according to The Sphere Handbook, FEMA, and Australian Red Cross. Physical distancing and hygienic standards were modified highlighting that ideal requirements are not always feasible; therefore, we may choose realistic recommendations for practical purposes and suspected cases of COVID-19.

WHO published guidance for clinicians and health care decision-makers on the use of corticosteroids in patients with COVID-19.
We recommend systemic corticosteroids for the treatment of patients with severe and critical COVID-19. We suggest not to use corticosteroids in the treatment of patients ...with non-severe COVID-19 as the treatment brought no benefits, and could even prove harmful. Treatment should be under supervision of a clinician.
Corticosteroids are listed in the WHO model list of essential medicines, readily available globally at a low cost. WHO encourages countries to maintain sufficient stocks of corticosteroids to treat COVID-19 and the other disease for which they are effective, while not maintaining excessive stocks which could deny other countries access.
