Ce document propose des listes de contrôle élaborées par l’OMS en matière de communication sur les risques et de participation communautaire (RCCE) pour la préparation et les premières ripostes face au nouveau coronavirus 2019 (2019-nCoV) r...écemment identifié à Wuhan, dans la province du Hubei, en Chine. Il a pour objectif de donner aux pays des conseils pratiques pour la mise en œuvre de stratégies efficaces en matière de RCCE qui contribueront à protéger la santé publique lors des premières phases de la riposte face à un nCoV. Ce document inclut des objectifs et des actions de RCCE recommandés pour les pays qui se préparent à faire face à des cas de nCoV et pour ceux qui ont déjà des cas confirmés d’infection par le 2019-nCoV.

Introduction The novel Coronavirus (nCoV) epidemic in 2019 -2020 has recently emerged. The route of transmission is not totally known, although it is known that it can spread from person to person, and local health care systems may be ill-equipped to handle a large-scale outbreak. Furthermore, misco...nceptions and misinformation about the disease often spreads rapidly in such epidemics.
In previous epidemics mental health and psychosocial support (MHPSS) has been identified as a key priority. MHPSS ensures the well-being of the affected populations, and counter-acts the threats to public health and safety that fear, stigmatization and misconception pose. Access to information, knowledge about the disease and how it spreads, make it easier for the affected to feel supported and calm, and to comply with instructions. Furthermore, psychosocial support to staff and volunteers help the operation as work conditions are extremely stressful.
This briefing note provides background knowledge on the MHPSS aspects related to nCoV and suggests MHPSS activities that can be implemented. The messages can be helpful for those in contact with patients or relatives and feel the strain of working and living during the epidemic. The briefing is aimed both at those working in any capacity with those affected by nCoV and for the MHPSS responders who implement MHPSS activities and interventions for everyone affected.

Le HCSP a fondé ses recommandations sur les connaissances actuellement disponibles en particulier les publications scientifiques relatives au COVID-19 mais aussi de recommandations relatives à la prise en charge des pneumonies post grippales, et des pneumonies sévères en réanimation.
Cet avi...s du HCSP délivre également des indications sur la prise en charge non spécifique, et de possibles thérapeutiques antivirales, dans l’attente de la mise en place d’essais thérapeutiques académiques afin de progresser dans la connaissance de la maladie. Ces recommandations relatives aux traitements spécifiques seront adaptées en fonction des résultats des essais cliniques lorsqu’ils seront disponibles.

El desarrollo de inmunidad a un patógeno por infección natural es un proceso de varias etapas que suele producirse a lo largo de una o dos semanas. El organismo responde a una infección vírica de forma inmediata a través de una respuesta innata no específ...ica, en la que los macrófagos, los neutrófilos y las células dendríticas frenan el avance del virus y pueden incluso evitar que este provoque síntomas. A esta respuesta no específica le sigue una respuesta adaptativa en la que el organismo genera anticuerpos que se unen específicamente al virus. Estos anticuerpos son las proteínas denominadas inmunoglobulinas. El organismo también genera linfocitos T que reconocen y eliminan las células infectadas por el virus: es lo que se conoce como inmunidad celular. Esta respuesta adaptativa combinada puede eliminar el virus del organismo y, si la respuesta es suficientemente intensa, puede evitar el agravamiento de la enfermedad o la reinfección por el mismo virus. Este proceso se mide a menudo a través de la presencia de anticuerpos en la sangre.

Migrant and displaced children are at heightened risk to the immediate and secondary impacts of COVID-19. They often live in cramped conditions with limited access to water, sanitation, and hygiene (WASH), may be in immig...ration detention or “left behind,” live with disabilities, unaccompanied or separated from their families, and can be hardest to reach with accurate information in a language they understand. Migrant workers and refugees can live in the most disadvantaged urban areas, where access to essential services is already limited. Refugee and migrant children may also be prevented from accessing essential services due to legal, documentation, linguistic or safety barriers. Further, the misinformation on the spread of COVID-19 exacerbates the xenophobia and discrimination that migrant and displaced children and their families already face.

Ces orientations vous proposent les principes et procédures de base en matière de décontamination de l'environnement physique dans le contexte de la pandémie du coronavirus 2019 (COVID-19). La contamination des surfaces et les risques de transmission des maladies diffèrent ...en fonction de l’environnement et le nombre de personnes fréquentant un endroit donné (par exemple, les établissements de santé, les transports publics et autres installations/lieux de travail rassemblant un grand nombre de personnes). Mais les principes de décontamination de l’environnement restent les mêmes. Etant donné que toutes les situations ne peuvent pas être couvertes ici, ces conseils peuvent être adaptés aux situations individuelles à mesure qu'elles se présentent. For COVID-19, as for many infectious diseases, the true level of transmission is frequently underestimated because a substantial proportion of people with the infection are undetected either because they are asymptomatic or have only mild symptoms and thus typically fail to present at healthcare fac...ilities. There may also be neglected or under-served segments of the population who are less likely to access healthcare or testing. Under-detection of cases may be exacerbated during an epidemic, when testing capacity may be limited and restricted to people with severe cases and priority risk groups (such as frontline healthcare workers, elderly people and people with comorbidities). Cases may also be misdiagnosed and attributed to other diseases with similar clinical presentation, such as influenza.
Differences in mortality between groups of people and countries are important proxy indicators of relative risk of death that guide policy decisions regarding scarce medical resource allocation during the ongoing COVID-19 pandemic. This document is intended to help countries estimate CFR and, if possible, IFR, as appropriately and accurately as possible, while accounting for possible biases in their estimation

Documents are divided into different sections, based on the ‘’spaces of new vulnerability” inheren...t to some IOM programmes although many of them are applicable to other areas. They cover both mainstreaming of MHPSS and specific actions.
MHPSS managers will also find guidance on how to address the less technical and more managerial and programmatic issues related with the pandemic, including programme redefinition, surge capacity and how to manage demands to provide staff support to colleagues in the same missions

This study shows the negative impact that the COVID-19 pandemic is having on access to HIV care in the Russian Federation and shows that people living with HIV in the country are more susceptible to COVID-19 but less likely to seek testing or treatment.
More than a third of people living with HIV who were surveyed reported some impact on HIV services, including about 4% who reported that they had missed taking antiretroviral therapy because they could not get the medicine and nearly 9% who reported that they had missed taking medicine for tuberculosis prevention. However, the majority of respondents (about 70% of people living with HIV) did not experience problems obtaining antiretroviral therapy and about 22% reported that antiretroviral medicines were delivered to their home. More than 900 respondents from 68 regions of the Russian Federation, including people living with HIV and those who are not, were reached by the study.
