The ... services delivery system in Zambia is pyramid in structure, with primary healthcare (PHC) services at community level, at the base, followed by first and second level hospitals at district and provincial levels, respectively, and third level (tertiary) services at national level. Notably, primary health services are free in Zambia and health service providers are either governmentowned or not-for-profit facilities.
Over the years, resource constraints have affected the quality and extent of healthcare services at all levels, requiring the mobilisation of additional resources for the sector. In doing so, prioritisation was high on the agenda of health sector reform. The EHB, therefore, prioritises interventions with the highest impact on the population, enabling policy makers to revisit priority diseases and conditions and to cost the services provided at each level of facility. Other key issues in developing the EHB in Zambia have included the need to have cost-effective services and cost per capita of services for more systematic budgeting, to rank interventions and to validate and cost the health benefit package as a whole. more

Each humanitarian setting provides distinct opportunities and challenges for actors to coordinate and collaborate at strategic ... operational levels. The Health and Protection Joint Operational Framework has been developed to ensure that the health and protection response during humanitarian emergencies can adapt to each environment and is adequately coordinated to ensure high-quality services to meet the needs of affected individuals and at-risk groups based on their situation or vulnerabilities. The Health and Protection JOF was conceived in 2019 as a collaboration between the Global Health Cluster (GHC), the Global Protection Cluster (GPC) and its Areas of Responsibility (AoRs), the Inter-Agency Standing Committee Reference Group on Mental Health and Psychosocial Support in Emergency Settings (IASC MHPSS RG), and the Inter-Agency Working Group for Reproductive Health in Crisis (IAWG), in addition to key technical experts. A Steering Group (SG) comprised of representatives from each of these entities guided the framework through a joint global analysis of good practices, gaps, and barriers to integrated and inter-sectoral response coordination. This included a mixed methods review of policy and practice, a survey of humanitarian experts, multiple case studies, structured stakeholder interviews, and field visits. This exercise produced a zero-draft which was then reviewed by field practitioners in three operational contexts to clarify and fully coordinate its operationally focused lens. Finally, the JOF was reviewed by the SG including via a series of consultations in early 2023 to consolidate the current framework. more
The 2019-2023 Strategy for UNU-IIGH, developed in 2018, built on UNU-IIGH’s strategic advantage and position vis-à-vis the UN ... global health ecosystem. The Strategy set a goal to advance evidencebased policy on key issues related to sustainable development and health and shifted the Institute’s body of work from investigator-driven global health projects to three priority-driven, policy-relevant pillars of work, each reflecting UNU-IIGH’s unique value position. When the COVID-19 pandemic hit in 2020, the Institute adapted and reprioritised its areas of work while continuing to deliver on the main strategic objectives of translating evidence to policy, generating policy-relevant analyses on gender and health, and strengthening capacity for local decision making especially in the Global South. The new strategic plan encompasses four work packages: 1. Gender Equality and Intersectionality: through this work, we will aim to improve the quality of health care through a human-centred approach, by ensuring the health system is responsive to the needs of structurally excluded individuals and communities; and by advancing a positive and enabling environment for the frontline health workforce—e.g. addressing the experience of gender-based violence. 2. Power and Accountability: through this work, we will catalyse equitable shifts in power and address key accountability deficits that prevent the equitable and effective functioning of the global health system and prevent adequate responsiveness to the needs of states and populations in the Global South. 3. To enhance health co-benefits across urban policies which tackle air pollution and climate change, WHO, in cooperation with various international, national, ... local partners, implemented the Urban Health Initiative (UHI) pilot project in Accra, Ghana. The Initiative prompted the health sector to use its influential position to demonstrate to decision-makers and the public the full range of health, environmental and economic benefits that can be achieved from implementing local emission reduction and energy access policies and strategies. Policy tracking, although not always considered, is a fundamental component of this procedure. It assesses the planning, implementation and progress of a policy to refine or adjust policies with the final objective of increasing the likelihood of the policy being successful. This report is an outcome of the last component of the UHI model process, Policy tracking and monitoring outcomes. The report proposes a framework for tracking urban health policies, with a special focus on the impacts of air quality and energy access on human health and well-being in African countries, giving some examples from the pilot project in Accra. The report also provides resources to survey air quality in cities and other tools to assess public health and the environmental impacts of urban policies and monitor or track their effects. more

During the first year of the Covid-19 pandemic, the world’s economy slowed. Yet, the global annual average particulate pollution (PM2.5) was largely unchanged from 2019 levels. At the same time, growing evidence shows air pollution—even when experienced at very low levels—hurts ... health. This recently led the World Health Organization (WHO) to revise its guideline for what it considers a safe level of exposure of particulate pollution, bringing most of the world—97.3 percent of the global population—into the unsafe zone. The AQLI finds that particulate air pollution takes 2.2 years off global average life expectancy, or a combined 17 billion life-years, relative to a world that met the WHO guideline. This impact on life expectancy is comparable to that of smoking, more than three times that of alcohol use and unsafe water, six times that of HIV/AIDS, and 89 times that of conflict and terrorism. more

The Event-based Surveillance Framework is intended to be used by authorities and agencies responsible for surveillance and .... This framework serves as an outline to guide stakeholders interested in implementing event-based surveillance (EBS) using a multisectoral, One Health approach. To that end, the document is arranged in interlinked chapters and annexes that can be modified and adapted, as needed, by users. This is a revised version of the original “Framework for Event-based Surveillance” that was published in 2018. This framework does not replace any other available EBS materials, but rather builds on existing relevant or related documents and serves as a practical guide for the implementation of EBS in Africa. This framework is aligned with the third edition of the WHO Joint External Evaluation for the following indicators: strengthened early warning surveillance systems that are able to detect events of significance for public health and health security (Indicator D2.1); improved communication and collaboration across sectors and between National, intermediate and local public health response levels of authority regarding surveillance of events of public health significance (Indicator D2.2); and improved national and intermediate-level capacity to analyse data (Indicator D2.3). As countries begin to implement and demonstrate EBS functionality they will ensure an increase in JEE scores and progress towards meeting the requirements outlined in the IHR3F Additionally, in African Union Member States that have adopted the Integrated Disease Surveillance and Response (IDSR) strategy, this document is a complement to and can enhance the implementation of IDSR, especially for the 3rd edition (2019) that includes components related to EBS. more
