Heart failure with a reduced ejection fraction (HFrEF) is a condition frequently encountered by healthcare professionals and, in order to ...chieve the best outcomes for patients, needs to be managed optimally. This guideline document is based on the European Society of Cardiology Guidelines for the treatment of acute and chronic heart failure published in 2016, and summarises what is considered the best current management of patients with the condition. It provides information on the definition, diagnosis and epidemiology of HFrEF in the African context. The best evidence-based treatments for HFrEF are discussed, including established therapies (beta-blockers, ACE-i/ARBs, mineralocorticoid receptor antagonists (MRAs), diuretics) that form the cornerstone of heart failure management as well as therapies that have only recently entered clinical use (angiotensin receptor-neprilysin inhibitor (ARNI), sodium/glucose cotransporter-2 (SGLT2) inhibitors). Guidance is offered in terms of more invasive therapies (revascularisation, implantable cardioverter defibrillators (ICDs) and cardiac resynchronisation therapy (CRT) by implantation of a biventricular pacemaker with (CRT-D) or without (CRT-P) an ICD, left ventricular assist device (LVAD) use and heart transplantation) in order to ensure efficient use of these expensive treatment modalities in a resourcelimited environment. Furthermore, additional therapies (digoxin, hydralazine and nitrates, ivabradine, iron supplementation) are discussed and advice is provided on general preventive strategies (vaccinations). Sections to discuss conditions that are particularly prevalent in sub-Saharan Africa (HIV-associated cardiomyopathy (CMO), peripartum CMO, rheumatic heart disease, atrial fibrillation) have been added to further improve clinical care for these commonly encountered disease processes. more

The article explores the underlying factors influencing unhealthy diets and sedentary lifestyles among ...dolescents in Kilifi County, Kenya. Using a qualitative approach, the study involved interviews and focus group discussions with adolescents, stakeholders, and young adults. Key findings include a preference for unhealthy, carbohydrate-rich, and sugary foods over traditional and nutritious options, exacerbated by factors like low socioeconomic status, urbanization, and poor farming practices. Sedentary behavior, such as gambling and extensive technology use, was prevalent, often replacing physical activity. Protective factors like school attendance, community-based services, and parental engagement were identified as mitigating risks. The study highlights the importance of ecological intervention strategies targeting intrapersonal, interpersonal, and community factors to address unhealthy behaviors and promote better health outcomes in adolescents. more

Asthma is the most common chronic respiratory disease (CRD) worldwide and is estimated to affect 262 million causing significant mortality ...nd morbidity, and has emerged as an important public health problem in many Latin American (LA) countries over the last 30 or so years. LA is a highly diverse region in terms of geography, climate, wealth, and ethnicity including 20 different countries with 639 million inhabitants, where 40 million are estimated to have asthma. A common feature of LA countries is the high level of social inequalities3 (Figure 1). In LA, asthma prevalence in both children and adults is highly variable and, where high, is among the highest worldwide, particularly in coastal tropical cities. more
Over-plagued with deadly siege, the Eastern Ghota is still subject to a vicious military regime attack in an ...ttempt to force its people to flee for their life or just die in place. The sufferings in Eastern Ghota have been continuing, unabated for more than five running years, leaving the area bereaved of almost all basic necessities for survival with grave shortage of resources. The Syrian regime is currently making attempts to displace the civilians by wreaking havoc in the area, preventing the entry of basic foodstuffs, and denying medical access for critical cases or even their exit for treatment. In a series of reports issued by the Information Management Unit (IMU) of the Assistance Coordination Unit (ACU) and through its network of enumerators and in coordination with the Local Council of Rural Damascus Governorate, the 2nd edition of the report ” Siege and Death in the Eastern Ghouta” is issued to sound off warning bells about the hardship situation in Eastern Ghota, much as it is intended to raise the alarm regarding the displacement enforced and the human rights violations committed by the regime. The report at the same time sheds lights on the looming catastrophe before it may occur. more

Está dirigido a los profesionales de la salud que se dedican al tratamiento de pacientes oncológicos pediátricos y que, directa o indirectamente, deben enfrentarse con las complicaciones que pueda causar el tratamiento a todos los niveles. Su contenido puede contribuir a la realización de un diagnóstico más certero de las alteraciones de la cavidad oral, así como al desarrollo de estrategias para la prevención y el tratamiento de estas manifestaciones.
