Pests and disease epidemics negatively impact the quality, quantity and safety of our food sources, and cripple economic growth and efficiencies in production. Furthermore, the epidemic and endemic levels of the pathogens and disease vectors can be difficult to control. This is why FAO stresses and promotes the special efforts required for cost-effective preventive measures rather than the more expensive control, disinfestation, treatment and disposal measures. When preventive measures are late or difficult, preparedness and contingency plans must be in place to enable rapid response. Early warning systems, based on close monitoring, surveillance, and timely reporting are fundamental to warn and empower communities to safeguard their livelihoods and assets by enhancing disease and pest prevention measures and for government services to take immediate measures to protect communities and national economies.

During the course of their work, health care person-nel perform clinical procedures or other activities that can expose both them andtheir clients to potentially infectious microorganisms. Many of their clients aresick and thus may be more susceptible to infections or may have infections thatcan be transmitted to others. Fortunately, all staff working at health care facilities can perform simple proce-dures to minimize risk—to themselves and clients—and reduce the spread ofinfections. These practices can be integrated at minimal cost into the routineworkday at clinics and hospitals around the world. This reference booklet isspecifically designed for use at all levels of the health care system, from thelargest hospitals to the smallest dispensaries or health posts, in settings whereresources are scarce. This booklet, which was first published in 1999, has now been updated. Whilemost practices remain the same, there have been a few important changes—forexample, in recommendations related to hand hygiene and standard precautions.Nonetheless, this booklet continues to present practical recommendations forsimple and relatively low-cost procedures that can be implemented anywhere,with basic supplies and little to no high-technology equipment.

Le personnel de la santé joue un rôle primordial, en ce qui concerne la protection de leurs clients contre les maladies infectieuses et leurs traitements en cas d’infection. Dans le cadre de leur travail, le personnel de santé accomplisse des tâches cliniques ou autres activités susceptibles de les exposer ainsi que leurs clients aux microorganismes – orga- nismes potentiellement infectieux. Bon nombre de ces clients sont malades et, par conséquent, sont plus vulnérables aux infections ou peuvent avoir des infec-tions susceptibles d’être transmises à d’autres personnes. Par chance, l’ensemble du personnel travaillant dans les services de santé est en mesure d’exécuter des procédés simples visant à minimiser le risque – vis-à-vis d’eux-mêmes et de leurs clients – et réduire la propagation des infections. Ces pratiques peuvent être intégrées à moindre frais dans les habitudes quotidiennes au sein des établissements de santé du monde entier. Ce guide de référence est spécialement conçu pour l’emploi à tous les niveaux du système sanitaire, depuis les plus grands hôpitaux jusqu’aux plus petits dispensaires ou centres de santé, dans des établissements où les ressources sont rares.Ce guide, publié pour la première fois en 1999, a été mis à jour. ressed in mg/kg of estimated biomass, were compared at country and European level. Substantial variations between countries were observed in both sectors. Estimated data on AMC for pigs and poultry were used for the first time. Univariate and multivariate analyses were applied to study associations between AMC and AMR. In 2014, the average AMC was higher in animals (152 mg/kg) than in humans (124 mg/kg), but the opposite applied to the median AMC (67 and 118 mg/kg, respectively). In 18 of 28 countries, AMC was lower in animals than in humans. Univariate analysis showed statistically-significant (p < 0.05) associations between AMC and AMR for fluoroquinolones and Escherichia coli in both sectors, for 3rd- and 4th-generation cephalosporins and E. coli in humans, and tetracyclines and polymyxins and E. coli in animals. In humans, there was a statistically-significant association between AMC and AMR for carbapenems and polymyxins in Klebsiella pneumoniae. Consumption of macrolides in animals was significantly associated with macrolide resistance in Campylobacter coli in animals and humans. Multivariate analyses provided a unique approach to assess the contributions of AMC in humans and animals and AMR in bacteria from animals to AMR in bacteria from humans. Multivariate analyses demonstrated that 3rd- and 4th-generation cephalosporin and fluoroquinolone resistance in E. coli from humans was associated with corresponding AMC in humans, whereas resistance to fluoroquinolones in Salmonella spp. and Campylobacter spp. from humans was related to consumption of fluoroquinolones in animals. These results suggest that from a ‘One-health’ perspective, there is potential in both sectors to further develop prudent use of antimicrobials and thereby reduce AMR.

Los SEM prehospitalarios también incluyen otros componentes como los centros tipo 911 o los Centro Reguladores de Urgencia y Emergencias (CRUE) y los programas de primer respondiente. Todos ellos deben integrase de una forma coordinada con las redes integradas de servicios de salud para asegurar una continuidad de los cuidados de salud prestados a la persona herida o enferma. Durante emergencias de salud pública, los servicios de emergencia medicas prehospitalarias pueden verse superados por el número de llamadas o demanda de traslados médicos.

ance of antimicrobial resistance in foodborne bacteria” is defined as the collection, validation, analyses and reporting of relevant microbiological and epidemiological data on antimicrobial resistance in foodborne bacteria from humans, animals, and food, and on relevant antimicrobial use in humans and animals. Integrated surveillance of antimicrobial resistance in foodborne bacteria therefore includes data from relevant food chain sectors (animals, food and humans) and includes data on both antimicrobial resistance and antimicrobial use. Integrated surveillance of antimicrobial resistance for foodborne bacteria expands on traditional public health surveillance to include multiple elements of the food chain, and to include antimicrobial use data, to better understand the sources of infection and transmission routes.
