This document updates recommendations for HIV testing by laboratories in the United States and offers approaches for reporting test results to persons ordering HIV tests and to public health authorities. The recommended algorithm is a sequence of tests used in combination to improve the accuracy of ...the laboratory diagnosis of HIV based on testing of serum or plasma specimens. The updated recommendations also include tests for HIV antigens and HIV nucleic acid because studies from populations at high risk for HIV demonstrate that antibody testing alone might miss a considerable percentage of HIV infections detectable by virologic tests

The guidelines acknowledge that overcrowding, unhygienic conditions and high inmate turn over contribute to the spread of infectious diseases within correctional facilities. The document states that voluntary HIV counselling and testing must be offered to all inmates when they enter facilities, duri...ng their incarceration at an inmate’s request and upon their release. All inmates must be screened for TB symptoms upon entry to facilities and at least bi-annually thereafter as well as upon release. The revised guidelines present two major changes to existing guidelines: (A) there are now just 2 categories of pneumonia instead of 3 (“pneumonia” which is treated at home with oral amoxicillin and “severe pneumonia” which requires injectable antibiotics) and (B) oral amoxicillin replaces o...ral cotrimoxazole as first line treatment, preferably in 250mg dispersible tablet form, twice daily for five days which can be reduced to three days in low HIV settings.
